Choosing a healthcare practice is an important decision. Here are answers to common questions about care, appointments, payment, and Premier Membership at Premier Advanced Practice Group.

Premier Advanced Practice Group is a self-pay practice and does not bill health insurance for visits or membership fees. This allows us to provide transparent pricing and focus on your care without insurance-related restrictions. Patients may still use their health insurance for outside services such as laboratory testing, imaging, medications, specialist care, and other services when applicable.

Premier Membership is designed for patients who want Premier as their ongoing primary care home. For $149 per month, membership includes routine primary care visits, preventive health planning, chronic condition and medication management, review and discussion of laboratory results, personalized health goals and care planning, ongoing follow-up and progress monitoring, and coordination of referrals and outside care when needed.
Laboratory testing, imaging, medications, specialist services, and other outside services are not included unless specifically stated.

No. Membership is not required to receive care at Premier Advanced Practice Group. Patients may choose individual self-pay visits based on their needs.
Premier Membership is intended for patients who want an ongoing primary care relationship with regular follow-up, preventive planning, chronic condition management, and continuity of care. Individual visits are available for patients who prefer to pay per visit

Appointments begin with an appointment request through our website. Select the visit type that best fits your needs and submit your preferred date and time. New-patient requests are reviewed by Premier before the appointment is confirmed. You will receive confirmation and additional instructions after your request has been reviewed.

Premier reviews new-patient appointment requests to help ensure that the practice and requested service are appropriate for your healthcare needs. In some situations, we may contact you for additional information or recommend a different level or type of care before scheduling.

Premier provides adult primary and preventive care, including routine health maintenance, hypertension, high cholesterol, prediabetes and diabetes, weight and metabolic health concerns, thyroid disorders, medication management, preventive screenings, and other appropriate primary care needs.
Some conditions may require referral to a specialist or a higher level of care.

Yes. Premier offers individualized weight and metabolic health evaluation and management. Care may include assessment of medical and lifestyle factors, laboratory review when appropriate, nutrition and activity counseling, treatment planning, and ongoing monitoring.
Medication may be considered when clinically appropriate, but prescribing is never guaranteed and is based on an individualized clinical assessment.

Laboratory testing is not included in the visit or membership fee unless specifically stated. When laboratory testing is medically appropriate, Premier can order recommended testing. Charges from the laboratory or other outside facility are separate from fees paid to Premier.

No. Prescription and over-the-counter medications are not included in Premier's visit or membership fees. Medication costs are paid separately to the pharmacy or other dispensing provider.
Prescriptions are provided only when clinically appropriate following an appropriate evaluation.

In many cases, yes. Although Premier does not bill health insurance for its professional services, patients may use their health insurance for eligible outside services such as laboratory testing, imaging, prescriptions, and specialist care, subject to their individual insurance plan.
Patients are responsible for verifying coverage, network participation, deductibles, copays, and other costs with their insurance carrier.

Yes. When specialty evaluation or treatment is medically appropriate, Premier can help coordinate referrals and outside care.
Because insurance plans have different referral and network requirements, patients are responsible for confirming that a specialist or facility participates with their insurance plan and whether additional authorization is required.

Telehealth may be available for selected established-patient concerns when clinically appropriate. Some conditions require an in-person examination, testing, or a higher level of care, and Premier will recommend in-person evaluation when necessary.

Prescription refills are reviewed based on clinical appropriateness, medication monitoring requirements, and whether appropriate follow-up has been completed. Some medications may require an appointment, laboratory testing, or other monitoring before a refill can be provided.
Please request routine refills before your medication runs out. Prescription refills are not guaranteed without appropriate clinical evaluation.

Appointments should be canceled or rescheduled as early as possible. Late cancellations, missed appointments, and rescheduling are subject to Premier Advanced Practice Group's current cancellation and payment policies.
Any applicable fees will be disclosed in the practice's patient policies and agreements.

Premier Membership requires a valid payment method for recurring monthly payments. If a payment is unsuccessful, you may be asked to update your payment information. Continued nonpayment may result in suspension or termination of membership in accordance with the Premier Membership Agreement.

Yes. Membership may be canceled according to the cancellation terms contained in the Premier Membership Agreement. Patients should review the agreement for information regarding cancellation timing, outstanding balances, recurring payments, and when membership benefits end.

No. Premier is not an emergency medical service and should not be used for potentially life-threatening or time-sensitive emergencies.
For symptoms such as severe chest pain, severe difficulty breathing, signs of stroke, loss of consciousness, severe bleeding, or another potentially life-threatening condition, call 911 or seek emergency medical care immediately.

No. Premier provides primary, preventive, and appropriate outpatient care. Some illnesses and injuries require immediate testing, imaging, treatment, or monitoring that cannot safely be provided in an outpatient primary care setting.
When necessary, patients will be directed to urgent care, an emergency department, a specialist, or another appropriate level of care.
